Summer Camp Forms and Payments

To comply with Montgomery County regulations, in order for your child to be able to attend our summer camp, please, fill out all the forms listed below and return them to us by June 5th, 2018.
The two following forms are currently only necessary for children who take the bus to come to camp:
- Bus Code of Conduct
- Bus Transportation Plan
Additionally, for children who do not attend a school located in Maryland, we have to have a copy of their immunization records on file.
Once completed and signed, you can either scan them and email them back to us at office@myfrenchclasses.org or send them back by mail at

This document is for your information and you do not need to return it signed (It aims at presenting to the campers and their families the general mode of operation of the camp, as well as expectations and obligations that we require all participating parties to accept).
